    Every clinic has seen it. The fee is due, the patient hands over a large note, the front desk has no change, and the patient says, "I'll just get change and come back." Some do. Some pay on the next visit. Some never do, and nobody writes it down.

    Each case is small. But it happens on busy days, when nobody has time to chase it, and the missing money never shows up anywhere because it was never recorded. Once they are home, a small pending fee is easy to forget, and awkward to bring up at the next visit. Some patients are short of money that day and feel embarrassed to say so.

    None of this is about bad patients. It is a front-desk gap: the clinic had no way to take the payment at that moment, and no record that it was still due.

    The cost is not only the fee. Your receptionist has to keep change, count cash at night, and remember who still owes what. That is a lot to carry in a register or in someone's head.

    Make UPI the easy option

    Many patients already pay for groceries, fuel and phone bills by UPI. Offering it at the counter removes the "no change" problem.

    Get a merchant QR code

    Your bank or a UPI app can give you a QR code linked to the clinic's current account. Before you sign up, ask about any charges and how quickly money reaches the account. A business QR code in the clinic's name looks more trustworthy than a personal number, and it keeps clinic money separate from personal money, which makes your accountant's work easier.

    Put it where patients pay

    Print the QR code and place it at the front desk, with a second copy at the billing counter if you have one. A small sign helps: "UPI accepted. Scan to pay." Many patients will pay without being asked.

    Confirm the payment before the patient leaves

    This is the step clinics skip most often. A patient showing a "payment successful" screen is not the same as the money arriving. Ask the receptionist to check the clinic's UPI app, or use a payment speaker from your bank that announces each payment aloud. If a payment fails, the patient can try again on the spot.

    Keep cash as an option

    Some patients, often older ones, prefer cash, and that is fine. Keep a small float of change at the start of each day so the front desk is not caught out in the evening rush. The goal is to give every patient a way to pay before they leave, not to force anyone onto UPI.

    Record every payment, the same day

    Taking the payment is half the job. The other half is writing it down. If payments are not recorded, you cannot tell whether a patient paid, what the day's total should be, or whether the cash drawer matches.

    • Bill every visit. Every consultation and procedure gets a receipt, even when the fee is small.
    • Note how it was paid. Cash, UPI or card, next to each receipt.
    • Write down anything still due. If a patient genuinely cannot pay today, note their name, phone number and the amount, and settle it at the next visit.
    • Match at closing time. Cash in the drawer should equal the cash receipts, and UPI received in the app should equal the UPI receipts. Any gap is found the same evening, not at the end of the month.

    Talking to patients about it

    Most patients welcome the option. A simple line at the counter works: "You can pay by UPI here, just scan this." For regular patients who have always paid cash, present it as a convenience, not a rule. If someone struggles with the app, the receptionist can help, or they can pay cash.     Tax and accounts

    When clinic payments go into the clinic's own account and every receipt is recorded, your accountant has a clean trail to work from. Keep each month's receipts together with the bank or UPI statement for that month. If you are unsure which records you need to keep for tax, ask your chartered accountant.
